Therefore we offer 4 different payment methods for you to choose from. Let us know which one works best for you! *Please note, as your schedule of care decreases, so will your payments Automatic deduction (Visa, Master Card and debit cards accepted) This is the most convenient payment plan offered. You may choose when you pay your balance (weekly, every 2 weeks, 1 time a month). On the day(s) agreed upon, an automatic payment will be made from your Visa, Master Card or debit card that will pay off what ever balance is owed (unless other arrangements have been made). Once supplied with your card number, we will keep it secure for your automatic payments only. Co-pays (Cash, check, Visa, Master Card and debit cards accepted.) Co-pays are collected on the day of each office visit. If you choose, you may also pre-pay for the week's worth of co-pays prior to your office visits. Upfront (cash, check, Visa, Master Card and debit cards accepted) This payment option is offered for non-insured patients (or patients who opt out of using their insurance). Pay for your entire program of care (base camp, uneven terrain or the summit) in advance and receive 10% off (A savings of $95 for your initial program of care). Insurance We will gladly check your benefit coverage. We accept assignment from insurance companies on a selected basis. If for any reason, the insurance company does not cover a service provided, it will become the responsibility of the patient to cover those charges. The Balanced Spine will offer any paperwork needed to assist you in getting reimbursed from your insurance company if applicable. Additional insurance information |
